2021 statement of liability received out of the blue

Just posting this for anyone that might've had a similar experience. I've just received an unprompted Statement of Liability from Revenue for 2021. I'm sure I made a return at the time and the balance was cleared, but my statement now shows a liability in the hundreds of euro - all of the liability relates to an adjustment on the statement, entered as 'U/P Coded' on their system.
Has anyone come across this before or had a statement out of the blue like this? I've submitted a query through their portal but this might resonate with the past experience of others here.

Did you receive or your employer receive any Covid supports through payroll in 2021? Can you see your previously submitted one to see what is different on this one?

Yes! The same thing happened to me in May this year, also for 2021. I'm a fairly standard PAYE employee in the public sector.
The only thing different is that the amount has been added into Panel 7A (Adjustments) as U/P Coded. There is no explanation as to how I underpaid as a PAYE employee, just that they will be taking this off me over the next four years.

No Covid supports, no stock options. Be interesting to see what they come back with on this.

Unfortunately the prior 2021 Statement isn't available to view, but as this new liability matches the U/P Coded amount I suspect that is the only difference to the statement at the time.

Did you change roles in the last few years? I had two separate instances where on changing roles the tax credit cert did not bring in my prior earnings, but did bring in my tax credits as if I had not used them so I ended up underpaying tax. I only realised when I submitted a claim for health expenses and got an underpayment.

What did you do, or did you chase them on this?
I'm afraid I did nothing in the end, so I'm very interested to hear what their response is to you! Are you sure the previous SOL isn't there? My previous ones are in My Documents > 2021 (along with the new 2021 one from this year).

No change, same employer from 2020 through until a week ago. I don't know if that (finishing up) triggered anything but it shouldn't have impacted on my position in 2021 anyway.

PAYE/myAccount/Form 12 or self-assessed/ROS/Form 11?
Good question - I was Form 11 until around 2021 (dividend income on savings taking me over the unearned threshold of €5k). It's been FOrm 12 since then though. Returns were made throughout and weren't materially different (I just have less unearned income since then).

Until and since when exactly? Were you liable for self-assessment in 2021?

You're right, the link to my SOL 2021 it brought me to My Documents, which I had understood to be a dud link. But I can now see my version of the SOL from 2022 (when I made my return) and the one supplied to me this week, with this 'U/P coded' being the only difference*.

*Some note and font changes in the years in between but nothing substantive.
