What kind of insulation is appropriate for a flat roof extension? Any thoughts appreciated?
 
Warm roof or cold roof?
I went with a warm roof - i.e. insulation is just under rubber so have no issues with ventilation or condensation.

I've put a deck/balcony on top of the roof - so I put plywood on top of the beams, then the TR21 and then plywood on top. Covered whole lot with single sheet of neoprene - Advance Butyl in Portlaoise. Delighted with it - fantastically warm.

If you're going with a cold-roof - you could just use glass fibre between joists - or Aerodorm - which is great as its easy to install from underneath.
 
I used extruded insulation as recommended by the ashfelter.
Its sits on top of everything and is weight down by pebbles.
Its wet and dirty but seems to work. Three layers is about 140 mm thick so you'll need dept and upright 'walls' around it of at least 200mm in height.
