What happens when an ill person has no family

What happens when someone who has no known family is ill in hospital & can no longer return to live in their own home? I presume "the state" take over but in real terms what actually happens? Talking about someone who wouldn't own their own home & wouldn't have much money.
 
It all depends.

If they don't own their home, i would imagine that as soon as a stay in a long-term care facility or (in the case of terminal illness) their hospital/hospice is arranged, a social worker or family friend or both would help the person clear up all the loose ends such as ending leases, paying final bills and organising to store or get rid of possessions that are no longer needed.

Are you worried about any one thing in particular?
