Ryanair refund not working

The flight has been cancelled but the online refund application will not verify - the following message comes up:

Unfortunately we cannot process your application for one of the following reasons:

• The customer name submitted does not match the name in the reservation check the number and re-enter

I've checked and resubmitted multiple times to no avail.

Any ideas?
