A PAYE worker receives €2,000 gross in 2013 for some work that has nothing to do with his PAYE job. Does he have to register as self-employed and fill out a Form 11?

As far as I know this is just additional income and should be included in your tax return upon which the Revenue will issue a bill. This is the exact same process as claiming extra relief for medical expenses and the like only in reverse!

Thanks. The Form 12 is almost as long as the Form 11! I don't even think Form 12 is available online. Can the income be coded through the tax credits system instead?

Form 12 is how you get it coded against the tax credits.

I.e. you file the form 12 (and yes the form is long, but if your only income is a P60 and the other €2k then the vast majority of the form is not applicable to you), and then the additional €400-€900 of tax will normally be collected by reducing your tax credits for the following year or 2.
