Non resident - Which to use - Form 11 or Form 12?

I have a simple question which I have spent a number of hours trying to get answered (both on this site and on revenue.ie) without success. What I am trying to determine is which form I should be using to submit my tax return for 2005 (i.e. the return that is due now before 31 October).
My circumstances are:
I lived overseas for 10 years and returned to Ireland in August 2005.

Therefore I am non resident and ordinarily non resident for the 2005 tax year. I was in PAYE employment for the last 5 months of the year and had rental income from an Irish property through the entire year.

I have received a Form 11 from Revenue and this is the form that I had been submitting in previous years to declare my rental income. However as I was a PAYE employee also for a few months in 2005, does this mean that in fact I should submit a Form 12 for that year and include the details of my rental income on it?
(If the latter, how/where on the form do I indicate my residency status and how do I avoid paying PRSI on my rental income while I was non resident?)
