A relative who has a medical card wishes to switch doctor she gets to visit for free with her medical card. How does somone switch their medical card to a different doctor?

i would think she has to get a new form filled in and re-apply under the new doc's name

Hi I think she needs to ask the health board for a "change of doctor" application form. She will then need to get the new doctor to fill this in as acceptance of the new patient. Then send it back to the health board. Hope this helps.

You have to be accepted by the given Doctor on to their medical card list. This is easier said than done.
