Hi,
I was on maternity leave from Dec 2008 to june 2009 and during this time i had an accident. I have since been let go from my job because of this but thats a whole other story! I went straight onto illness benefit in June 2009 and I have now exhausted my entitlements as my 2 years are up. I am waiting for surgery again. I was with my doc last week and I have a totally separate illness now and he gave me the form as if I was making a fresh claim for a different illness but I am unsure if i can do this. I dont know what to do now, please can someone advise me.. Thanks
