ICB report

I received my up to date ICB report today and it's showing 1 item that is scheduled to be removed in January next. Last June I got an ICB report and it had just one item on it which was listed for removal in Oct 2016 and is not now on the new report. It didnt list the one for removal in Jan 2017. I've figured out the reason this wasn't on the June report is that I had not included my maiden name in my original search and did include in my recent one. Basically I'm wondering if I was to apply for any kind of credit, not that I'm intending to, would I need to provide my maiden name or would the icb search be done on my current name? Or what extra information is given to lending institutions that I wouldn't get on an ICB report?

I'd love to see my ICB report but i'd hate making the ICB €6 richer for the privilege.
I'd imagine it's fairly colourful.
