ICB : Mortgage reported twice by lender affecting credit rating

I recently attempted to get a motor loan from the credit union, but I have been stopped in my tracks by a duplicate transaction on our ICB credit report. I ordered it last week and it appears that ptsb has registered the same mortgage twice on both my file and my wife's.

The first mortgage account on the ICB report indicates that we were in a moratorium, the 4 months in arrears and finally had terms revised. The second entry, quoting the same mortgage number, confirms repayments in order for the past 24 million months. I have tried unsuccessfully to get ptsb to explain why they have reported two mortgage accounts in my name. Now my credit union needs a mortgage statement from January to date, ptsb say this will take ten working days!

I have fired off a letter to ptsb, has anyone else had to deal with duplicate transaction on their credit report like this before?
