Homecarers credit address?

I am looking to claim homecarers credit for my wife who has given up work and cant seem to download the completed form online.
Does anyone know what address I can post it to instead as cant find it online?
 
Why don't you claim it via your Revenue MyAccount? No posting necessary. If you want to physically print a form, fill out our and post it to an address then just put your PPSN into the contact locator on the Revenue website to get the address to send it to based your circumstances.
