Have the yellow forms changed?

I work part time and have just changed over from full jobseekers benefit to claiming for the two days I don't work. I received a white form that needs to be signed off by the company. But I received ones out in the post that are yellow that only need to be signed me. Can anyone that is doing the same as me tell me what forms they are getting. Thanks.

Yes,no need to involve your employer.Just fill in the form sign it and drop it back in.If your days are the same every week,pre-fill in the four weeks at once.
