Getting a Letter of No Objection From Revenue

I'm trying to strike off a company and I have everything sorted bar the letter from revenue.
I initially thought you could apply online for it but now I see that you have to apply for it in writing and send it to "Office of the Revenue Commissioners, National Companies Unit, PO Box 1, Wexford"

I'm sure I could spend a few hours looking to see if I could find what needs to go in the letter but I thought I'd ask here and see if any of you has a "template" of sorts that I could fill in the relevant details for me or even if there is a simpler way of doing it online first.

I think there's a Revenue Tax and Duty Manual somewhere that explains exactly what's needed.

I think if you Google it, you should find it easily enough.
