This is reported in today's Irish Times, but I can't find it online. The thread title is the title of the story

A woman complained about her treatment by AIB
The Ombudsman rejected her complaint
FLAC wrote to the Ombudsman asking him to review his decision as it was wrong in law.
The Ombudsman applied to the High Court to quash his decision

I don't understand the original complaint, but the significant issue here is that the Ombudsman admitted he was wrong and applied to the court to quash his decision. I had not realised he could do this

I am familiar with another case, where the Ombudsman upheld a complaint.
The bank appealed the decision to the High Court.
The Ombudsman's legal team advised the Ombudsman that his decision was wrong.
[On reading the papers, the initial Ombudsman decision was very flawed. He should never have upheld the complaint]
The Ombudsman did not defend the appeal and asked the High Court to overturn his own decision.
The High Court upheld the appeal.
The Ombudsman heard the case again, and rejected the complaint.
