Form for exiting partnership

I left an equity partnership and am PAYE now. When they Revenue they said I had to lodge a form. I've forgotten the number of the form. Can anyone remind me?

What was the form to do? There are a number of potential forms to be completed when leaving a partnership to PAYE. Are you a controlling director of a new entity? If so, you may have to apply for S1 status. This is done by writing to the revenue which can be done directly through ROS or to your local office.

I would contact the revenue and ask them what is required.

It may be a P46 that they require. This is usually done by your new employer. [broken link removed]
