I have this. I set it up via the Boots App. Once you "join the over 60's club" you automatically get the points anytime you use the card when paying. A word of warning, it dropped off my account twice and I had to add it back. The App is not very intuitive and using the points you accumulate is a bit odd.

Yes, my wife gets her points. It's the discount on purchases over €20. that I am unsure of. Are these given as extra points or do you get a discount off the actual price of something. Never seen it on any receipt over the years.
Heritage card has a discounted fee for age 60+.

If you're in Dublin then DCC Sports and Fitness Centres (pool and gym) have discounted fees for age 60+. (If you join then you can use any/all 4 centres).

Yes. €21 gives those aged 65+ full access to the pool/gym (and sauna/steam rooms where applicable) of the 4 DCC Sports and Fitness Centres for 12 months. I think that classes may require additional fees. But it's a great deal and, in my experience, many of the older folks (certainly in Ballyfermot) make great use of it.

Odeon Silvers at Odeon cinemas for over 60 's. Film . Coffee/tea and biscuit 2euro.

That reminds me - Light House Cinema does something similar for over 65s every Fri late morning/midday, new release film. A bit more expensive - 7 euro, free tea/coffee. Not sure if they ask for proof of age.
IFI Cinemas also has regular "Wild Strawberries" over 55 screenings at a reduced rate, usually an arthouse title.
