Disabled grant for house renovation

Planning a house extension. I have a relative who does not live with me who is confined to a wheelchair. I have heard that I could get a grant towards the house extension if I make it accessible for him when he visits. Is this true or does anyone know where I can get info on this? Thanks.

I'm not sure if you can get grants for renovation/improvement/modification of a house that is not the disabled person's own residence but this link might be of interest:
