Dental expenses - new year

Hi guys,
I have 5 appointments between Feb. and Mar. next year
with a periodontist. He estimates about €2000 total.
According to the revenue I can make a claim on this work.
Does he fill in the Med 2 and I then send that plus the Med 1
form in. If so when do I do this ?
