Confused about Form 11 or form 12

I need some help. I am a PAYE worker that is my main source of income. For the years 2010 and 2011 I earned dividends of 140.00e in 2010 and 470.00e in 2011.

In 2012 I earned a dividend of 920.00e and for the last three months of the year I began renting out a property that had previously not been let.

I have not made any returns for the above and really want to get everything in order with revenue. What is the best way to go about this?

Any advice appreciated.
 
I'd go for a Form 12 for 2011 and 2012 assuming your non PAYE income is less than the €3,174 threshold. You can print them off revenue.ie
 
Thank you. Would you do a Form 12 for 2010 as well? Just wondering can any of this be done through PAYE anytime as I have med form 1 to complete as well for each of those years.
