- Tick box for 'Not a specified US person'
- Tick box for 'Not a financial institution'
- Under 'non financial institution' tick 'Active non-financial foreign entity'
- tick box for 'Active non financial entity (NFE) other than those specified
- Provide country of tax residency 'Ireland' and enter tax id number
