Bath tap problem

Hi All;

I have a problem with my bath tap/shower hose.

The lever switches from Tap to Shower etc, however if I switch to tap, some water still comes out of shower head and visa versa.

I presume a washer is gone somewhere, but I already replaced the washers on each tap head, but I still have the same problem.

The head of the screw that holds in the level also came off, so any idaes where I would get such a thing?
