2008 income tax and PRSI refund

Hi,
I earned euro 2328.32 during the first couple of weeks of 2008 and paid euro 171.24 income tax and euro 129.53 PRSI. This was my full Irish income for the year after which I moved to a non EU country.

I believe I'm entitled to a refund of the income tax (Fill out Form 12) and also the 2% health levy and possibly the remainder of the PRSI.

Couple of questions:
How do I go about getting the remainder of the PRSI refunded
From whom do I get my P60 from for 2008 employer or government - (I received my 2008 P45 and 2007 P60 last week from my previous employer - would they also have the 2008 P60?)

as far as i know. you dont get a p60 from your employer for 08. the p45 will suffice. you had to be in employment at the end of the year to get a p60
 
To get the PRSI refunded, you need to apply to the local social welfare office
