What's PRSI payable on?

So I'm currently liable to PRSI and Health Levy as an employee under Class A.
What if I earn income from other sources. In Particular would I be liable to PRSI/Health Levy and if so, would it be at employee rates or self employed rates for:
Dividend income
Rental Income
income from a trade

From looking at the E&Y calculator [broken link removed], it appears that
Net Rental Income & Dividends & "Any other income" incur Health Levy but no PRSI.

But if that "Any other income" was self-employed income, then i would expect to be paying PRSI at self-employed rates

Any help appreciated
 
As far as I know any non PAYE income over c. €3,250 (?) becomes assessable for PRSI (not sure about health levy).

The non paye income limit for PRSI used to be €3,174 (IR£2,500). The health levy only applies when income is over approx. 20K - not sure of exact amount.
