I pay BIK on my VHI per my payslip. I am a propiretary director of the company, can I claim the 20% credit by entering the gross premium in my personal form 11?

I received a list of directors fees from the company accountant with VHI total for the year on it. He said to add to my gross salary on my personal form 11. Have I not already paid tax on the VHI as BIK in my payslip?

Yes, you should claim the 20% credit when submitting your income tax return.

You've already paid the tax but it should still be reflected in your income tax return. The tax you've already paid will also be reflected so no additional liability should arise.
