Top up mortgage - Self Employed

Can anyone give me advice. I am self employed and am going for a top-up on my mortgage. I need a letter from my accountant outlining this years results and projections for the next year. Has anyone ever done this? What else can I get written on the letter so the bank will look favourably on the request?

It will save you time if your accountant also confirms that all your tax, PRSI and VAT affairs are up to date and how long s/he has been your accountant.
