Tax Relief on Charitable Donations

I've been told by charities that I need to claim for tax relief on charitable donations myself as I fill out a fax return aswell as mainly being a PAYE worker.
I fill out a Form 12 but can't locate the section to claim this tax relief in the Form 12.
Does anyone know if I should be able to claim this using Form 12?
If not, rather than switching to another form e.g. Form 11, can I let the charity claim the tax relief if I can't do so using Form 12?

I think the form you are looking for is CHY2. You can search for it on http://www.revenue.ie/en/index.html

If you are a PAYE worker filling out Form 12, the charity can claim back tax on donations of EUR250 or more.
