Question on W-8BEN form

Hi,
I have some shares in the form of a certificate(held by myself) which are listed on the NASDAQ. I know I need to fill out a W-8BEN form and so that they will not be subject to any US witholding tax if I sell them.Where do I get such a form and to whom do I return it to?
