query about rent relief form

I am filling out a rent relief form for a room I rented in a house from August of last year to May of this year.

One of the things that the form requires is the tax year for which im making the claim, does this mean I have to fill out the same form twice with the same info with the only thing different being the years 2009 and 2010?
 
Also I can also claim for other places I have stayed in over the past 5 years ,however I do not have the landlords PPS numbers for thesw, is this a must?
 
I would fill in form twice

On the 2009 form enter date as 31.12.2009 ( for tax purposes )

You can claim 4 full tax years , so in your case - 2009 - 2008 - 2007 - 2006

Fill in the Landlords name and the address of the property - You do not necessarily need his PPS No.
 
Once you receive the rent relief, it is added to 'future' years as a default (you need to have it removed should your situation change as they 'assume' you're still in rented property). I'm not sure how this relates to retrospective applications though (I'd strongly assume it doesn't apply from a logic pov), so personally I'd fill in the form for each year in question to get the relief(s) asap.

The PPS number is requested (makes the paperwork easier) but not essential. Forms can and will be processed without it.
