Probationary Period is up - no contract yet

I'm looking for some advice regarding my current position at work.
I was taken on as a full-time employee but on an initial probationary period of 9 months with three reviews after each three-month period.

I received the first of these reviews four months into the job and all was well and good - very satisfied. The next review came nearly five months later and, again all was very positive.

I am now into my tenth month working for the company and as of yet have not reveived my final review - should I be worried? Also, seeing as the period has passed and my contract has not been terminated, do I take it that I'm now a 'permanent' employee?

Also, once made a permanent employee I would be entitled to health insurance. I have my own but thought I would not still be paying for it at this stage and its likely I will have to pay it for another month. What is my position on this?

Any help would be greatly appreciated. I'm very stressed about this and getting worse! Aaaaagh.
 
Have you asked you boss/HR where you stand on the contract?
Health insurance schemes are usually optional for employees but if it is standard that the company pays for its permanent employees then if you are made permanent you will get it.

In short; ask.
