Hi, I received a letter from revenue to pay and file. I am a PAYE worker and had no extra income outside this in 2008 or 2009. Do I complete Form 11 or Form 12? Do I need to include PAYE income in the returned form. I also had a MED1 claim for 2008.
 
If you had no other income in 2008 then I would contact Revenue & inform them of this.

You are not obliged to file a Form 11 for 2008.

You are if they ask you to and in that case, yes, you should enter all income (including your PAYE income) and your deductible expenses (such as medical costs).
 
In my experience, a simple phone call to Revenue informing them of the situation should ensure that you won't have to file the Form 11
 
if you are PAYE and have deposit interest earned do you have to pay and file. I know its taken a source but in theory.
