OPFA - What documents?

Am going to my local office tomorrow to apply for OPF and want to bring as much of the documentation with me as possible. Can anyone tell me what would typically be asked for?

I have the children's birth certs and 3 months bank statements.
I don't have a utility bill in my name but have changed the name on the account and should be able to supply one shortly.
I have been at home with kids for 4 years so don't have any employment forms.
We were not married so no separation documents.
I have seen a solicitor and asked her for a letter to say that I have taken legal advice.
I have no maintenance agreement/ order but have been in touch with the family mediation service and hope to sort something out in the next few weeks. I am on the list for cancellations in December, I could bring a copy of the emails.

I understand that I will be waiting some time to get this sorted out, but will be able to apply for an interim payment from the local CWO. Having opened the claim with the local social welfare office, how do I go about getting the allowance from the CWO?

Once you have applied, ask for a receipt or note from the SW office, bring it to the CWO and apply for Basic SWA. You can find the application form for SWA on the welfare website too.
