NTMA targeted in voice phishing attack

A garda investigation is under way after the National Treasury Management Agency was targeted in what is understood to be a €5 million voice phishing attack.

I guess we'll have to wait to see what exactly happened but it seems odd to me that their processes/procedures wouldn't be robust enough to prevent €5M being paid out on foot of an impersonated (AI?) voice call and a fake invoice?
