Make a hotmail a/c and then when installing MSN it should give you a step-by-step setup guide. (Bear in mind that most users seem to prefer Windows Live Messenger because of it's more advanced features.)

Basically go to www.passport.com and you can register any email address as a Microsoft ID which can be used with msn or windows messenger
