medical card & disability allowance question

Hi i am a mum to a 19yr old who is living with us & has been diagnosed with chronic psoriatic arthritis. both my self & my husband work i only work 15 hrs a week, my husband full time.. it has cost us a couple of thousand euro with hospital & clinic bills just getting our daughter diagnosed as we have no health insurance she was told to apply for a medical card & disability allowance as she will be on meds for the rest of her life & will require blood tests every month because of the meds she is talking.. she have limited mobility when her joints swell which is nearly all the time now.. she is working 12 hrs a week & loves her work & feels if she gives that up she will have no quality of life, she hopes to start college tomorrow.. will my husband & myself have to declare what we earn even though its my daughter who is applying for both medical card & disability.. really sorry for this been so long..
 
Persons aged 16-25 will be assessed on their own income only if they are deemed to be financially independent.

This means that if your daughter's income (her wages, disability allowance when it comes through and student grant if eligible) is equal to or greater than the single rate of SWA (currently €197.80 per week), she'll be means-tested in her own right rather than on your incomes.

If she does not meet the means test limits, she can appeal and, given the circumstances outlined, could be granted a card on "undue hardship" grounds - where her requirement for medical services would cause her financial and other difficulties without a medical card.

Her wages is just about the €197.80 give or take a little each week she have no college grant or any other payment only the part time job however long she may last in that:(
as i stated she is living with her dad & myself as she will not be able to live on her own as her balance is affected when the arthritis is at its worse.. do i fill the medical card application as she is not dependent on us as long as she is working or do i still have to put her dads & my earnings on the application as she is living with us??
 
The application form should be in your daughter's name, and should give all her income details (there's also a space for any other information, so some of her medical details could be included here if you wish).

As far as I recall, there's a section which asks who else is living at the address - the rest of the family details can go there with income details if asked (sorry, don't have an actual application form to hand, it's a while since I've seen one!).
