Lost P45 Need to make claim 2011

Daughter moved abroad in 2011 but never claimed any refund ( She left early Feb 2011). P45 is lost. Only have her PPS No. Should she use Form 12? Don't have employers no either.
Any help appreciated on this

Edit have found an old payslip with employer no.

As she left employment in Feb any refund would only be from January of that year. However if there is a refund due she should claim it.

Would that payslip you have be her final one, or her former employer may be able to supply a statement of earnings
 
She should just write to her local Revenue district requesting a PAYE Balancing Statement for 2011. She doesnt need either a Form 12 or a P45 for this, although Revenue may request either or both, either to resolve any gaps or issues on their own record in relation to her pay or tax details or that year, or otherwise simply to independently verify these details.

Simplest way to start it, is for her to phone the PAYE helpline and request her balancing statement, explaining she left the country mid year - or else write in with the same request.
