If sending in a letter to ask the ombudsman to conduct a review of the mortgage what do you include with the letter?

Any correspondence between yourself and bank?
Mortgage a/c number?
Mortgage contract?

You must first follow the complaints procedure of the bank to the letter.
If you are not satisfied:

First thing is to write a letter stating why you feel you should be put on the tracker, mentioning or quoting the relevant parts in the terms and conditions or letters of correspondance between you and the bank. Send copies of all the documents you quote or mention along with this letter to the ombudsman. Any supporting docs you miss or remember later can be forwarded at a later date when the ombudsman replies with a summary of your complaint. He will also send a response from the bank to him. Note that the bank will see all documentation that you furnish also at some stage in the process.
