Motor Insurance NCB oversight

I've been driving for some time without any claim. I recently had to renew and the renewal form states the following:

Are you entitled to NCB in your own name: NO
NCB Entitlement : 0 years
No. of years as named driver: 3 years

I am 3 years with my current insurer and I am the owner of the car with my wife as a named driver - No claims. Previous to this, we also shared a car with my wife as primary driver and I as a named driver with a different company. In both cases, over a total of 10 years, no claims exist.

As a named driver, can I carry NCB forward?
What reasons might exist for NCB = 0 above?

Have you made a claim, even for a windshield in the past 3 years?

Often this will reflect in these details, though if you asked for your official statement of No Claims I believe it shouldnt include this claim.
