How do you claim Medical Insurance Relief

Hi all, my employer pays for my VHI - I want to claim back medical insurance relief at the end of the year. Its very difficult to find how to do this from the revenue's website. Is there a form available to do this? I see from older posts that people have suggested that you write to the reveune - if this is the case what do you need to include in the letter and where do you post the letter to?

you send a letter or an email to your local Revenue office.

Inform them that your employer pays your VHI for you as BIK, and tell them the gross amount of the premium. They'll send you a balancing statement giving you the tax relief.
 
medical insurance relief now done at source so you should have all relief you are due already. This has been case for last 2 years (I think). Prior years certainly you need to write to revenue.

Sorry, that's not quite correct.

If your employer pays for your Med Insurance for you (as a Benefit-In-Kind) then you do not get Tax Relief at Source on it. You have to apply to Revenue to get the relief. The o/p said their employer pays their VHI for them.

Tax relief at source is only given automatically to individuals who pay it for themselves. This has been the case since April 2001.
