Form for claiming tax for an estate

As part of my dads estate there was a pension contribution stopped. We received a letter from his former employer stating that tax relief could be claimed on this pension contribution. Does anybody know what form the executor (my mam) should fill in to claim this tax back into the estate?
 
Hi. Condolences on your loss. A quick look at revenue.ie throws up this page http://www.revenue.ie/en/tax/it/deceased-persons.html#section1 . It seems you need to gather up all relevant info, letter from employer re deduction and last payslip/P60 etc and any other claimable expenditures, e.g medical expenses, tuition fees, private pension contributions etc and submit to your local tax office. They are quite quick and very helpful in sorting out this kind of issue.
