Background:
Married but taxed as a single person.
Home owned jointly with joint mortgage
Previous home (now rented) owned jointly with joint mortgage. Husbands PPS number used for rental income purposes.

Have never completed a Form 12 and received letter stating I was overdue to return 2 years

Couple of Q's

Do I select single for Form 12 as taxed as single person?
Husband has always included 100% of debt and rental income in his tax returns - do I also need to include?

Why have they asked you to fill in a form 12. It appears that your husband is declaring all the rental as his income so therefor the tax is up to date. Your form 12 may well be in connection with something such as ESPP and not the rent.

The OP is assessed as a single person, and if her name/PPSN is showing up on PRTB / NPPR records, then it may well appear to Revenue that she has a filing obligation... they have no means of knowing that the amounts returned on the husband's F12 necessarily equate to all of the rental income.

I took this to mean that all forms regarding rent are solely using the husbands PPR however as you say the OP's number may have appeared inadvertently somewhere along the way.
