Hi,
I am doing Form 12 for the first time in seven years that I have lived in Ireland. My position was made redundant in April'2009, following which I took up a new job in June-09. Consequently, I have two income levy certs and two sources of income. Which employer do I write in the section 3 of Form 12? If I write end-of-the-year employer (that issued the P60) do I add an appendix with previous employment details and income source?
Also, do I need to submit a separate form for claiming top slicing relief or "tick" on this form 12-section 6 will suffice?
Any help much appreciated.
