Form 11 - Attach cheque to form or not

Hi,

I think the thread name says it all. I've looked up the revenue website but can't find a clear answer

The form is to be returned to:

Office of the Revenue Commissioners,
Collector-General's Division,
PO Box 354,​

Limerick.

but does the cheque be sent with the form to the same address??

I've been told to put my PPS number on the back and state it for '2010 Income tax'

Also who exactly is the cheque payable to?

Any help is welcome

Payable to ' Collector General'. Send it with the Form 11. I always send a cover letter too and take a photocopy of clients cheques (just in case)
