Equal opportunities in the North - job application

Not a question so much as an OMG!

Sitting filling in a job application for a position up North and there was an equal opportunites form that needs to be filled in. It asked me for my DOB when down loaded it, I was thinking ok, I wouldn't normally give that with my job application, but if that's what they want...

Then I saw the form: Gender, religion, nationality, race, disability, maritial status, dependants and sexual orientation!!

I understand that the form is for statistics and not used as part of the selection process, but I with each question there was a little gasp. It was like a list of questions you would give to someone new to interviewing of questions not to ask.

When application arrives in office, admin person numbers each form, and separates them. Interviewing/Shortlisting panel NEVER sees Equal Ops sheet. It is purely used for monitoring purposes, and is necessary for many funders.
 
I went for an interview in Belfast once(my ex employers had offices both sides of the border). Before the interview I was "read my rights" by the interviewer in terms on Equal Ops and had to confirm that I understood them.

If you don't like filling these in then either leave it blank (but obviously this could jeopardise your application) or don't apply.
