Declaring and Paying tax on Dividends

Hi,
on the form 12 you are asked to declare gross income for the year. I just take this from my P60 (I am a PAYE worker).

Are dividends income and do I add them to the P60 figure or do I declare them separately?

How do I pay the income tax due on a dividend - what is the form I need to fill out?

Employment income goes in box 3, irish dividend income in box 18. If you are talking about Irish dividends tax at 20% will already have been deducted. If you pay tax on your employemnt income at 42%, a further 22% will be due on the dividend income.

And how do I pay this - do I just send off a cheque to the collector general?
Is tax on 2005 dividends due by October 31st like CGT?
