Are dividends income and do I add them to the P60 figure or do I declare them separately? How do I pay the income tax due on a dividend - what is the form I need to fill out? thxs

If you are talking about Irish dividends tax at 20% will already have been deducted. If you pay tax on your employemnt income at 42%, a further 22% will be due on the dividend income.
