Claim tax back on Medical Insurance

Can I claim tax back on Medical Insurance? I'm a PAYE worker and have a private policy with Quinn Healthcare. It is not a company scheme and I do not pay at source. If I am entitled to make a claim what form do I need to fill in? Thanks
 
Tax relief for private health insurance is allowed at the insurance provider side (i.e. at Quinn in your case). The amount you pay for your premium includes the reduction for tax relief due.
