changing names on title

Myself and my girlfriend may be splitting up. I wish to move out and buy a new apartment and leave the existing in her name. Can we change the title of existing apt to her name and allow me to move on and buy new as an owner occupier.
What are the legal implications here and can anyone recoommend any where for further information.
