"Agent Form" from Social Welfare

I have submitted an application for Disability Allowance on behalf of my son and he has now received a letter from Social Welfare including the "Agent form"

The first few pages are straightforward and appear to relate to collecting the payment (should he be awarded it) by an agent

However Part 6 is a Medical practitioner cert that should be completed if person is unable to manage financial affairs.

My son will never want to deal with social welfare directly and will be happy for me to deal with that, however I don't want to take ownership of all his financial affairs. Should we get this filled in or not? Will it help if I need to contact SW on his behalf?

 
At 16 his DA will transfer over too him completely and solely in his name.
It will also be payable too him.
If totally incapacitated you can get it put into your account but this might lead too issues regarding means tests etc.

Maybe have a look or give financial well-being a call who would be well able too advise you of the best options for both of you.

I think you may be reading too much into Part 3 of the form. It relates only to any payments that you will receive on your son's behalf from Social Welfare - not to any other financial affairs or assets that he may have.

It's a standard form that is issued to thousands of people annually. And if you don't complete it as requested, then SW won't be in a position to discuss your son's application and associated matters with you. Is there someone else who could deal with Social Welfare on his behalf if he's unable or unwilling to do so himself?

Yes i reckon im over thinking it. Im happy to deal with SW on his behalf . just the wording of it seemed to suggest more. Thank you

This is a new application. We dont get DCA or any other payment, so this is all new to us.

If that’s the case the group I mentioned earlier will clearly explain ALL the stuff that is available too both of you.
They also do free workshops which are very beneficial.

I was actually just at one their online workshops! Heard them mentioned on this forum and it was very informative. Im gonna hold off setting up a session as going to speak with a solicitor first regarding will.
I just had never heard of this extra form but seem to be pretty standard thankfully.
 
Just in case anyone is looking for similar info.
That form was standard and gets sent out regularly.
I brought it to my GP and she signed it.
This now allows me to deal with DSP on my sons behalf as well as allowing me to collect his DA in post office (until such time i have an appropriate bank account in place).
Definitely worthwhile to have this in place.
