I think he's referring to some plans that really only cover public hospitals and are way over priced.
For example, Aviva We Plan Level 1 with Day to Day A (1299pa) vs Health Plan 16.1 (1218pa).

We Plan Level 1 really only covers a shared ward in a public hospital, the shortfalls for private and hi-tech hospitals would be too expensive to even consider paying. Whereas Health Plan 16.1 is 81 euro cheaper per year and ticks all the boxes - from private room in a public hospital, all the private hospitals and hi-tech hospitals plus extra day to day cover. In other words, Plan 16.1 has double the cover compared to We Plan Level 1 and is cheaper - doesn't make sense but its true.
