form for claiming back pension tax relief

hi all,

recently made a 1 off pension contribution and have received a letter from administrator to say that I have made a pension contribution

what form is it I need to claim back the tax, I have about 4-5k to claim back, for the life of me I can't find it

As a PAYE worker, I normally use form12 which is a full tax return. Not sure if there is a separate one for just pension contributions...
